Forms for new and continuing clients to complete:

  • Pre-planning profile (prospective clients; 10 min. est. completion time).

  • Plan review form (continuing clients; 5-10 min. est. completion time).

  • Upload financial account statements (prospective clients, new and continuing clients; 5 min. est. completion time). Securely upload investment account statements (employer retirement plans, brokerage accounts, IRAs, mutual funds), 401(k) investment options, employee benefit summaries, insurance statements and/or summaries (not the entire policies please!), other relevant financial documents.

  • Upload your 2024 and 2023 tax returns (prospective clients, new and continuing clients; 5 min. est. completion time). Securely upload your 2024 and 2023 tax returns; forms 1040 and all filed schedules in .pdf format. Upload state and local tax returns also, if applicable.

  • Relatives form (continuing clients; 5 min. est. completion time). Don't enter your information or your spouse's information -- complete this form for each of your children (dependent and grown), parents (living and deceased), ex-spouses and grandchildren.

  • Complete the risk profile (prospective clients, new and continuing clients; 10 min. est. completion time). Complete the risk profile (if married, both spouses complete separately).

  • Upload estate documents (continuing clients; 5 min. est. completion time). Securely upload wills, trust documents, powers of attorney, marital agreements, and asset protection structure documents such as LLCs or FLPs.

  • Upload pay statement details (new and continuing clients; 5 min. est. completion time). Securely upload your last year-end and most recent payroll detail statements (for both spouses if married).

  • Provide details related to mortgages and other loans (if applicable): original amount and date, current balance, interest rate, term, monthly payment amount (P&I), additional monthly principal amount, escrow payment amount. This information can usually be found on your most recent statement. Also include annual homeowners insurance and maintenance costs.

  • Provide a budget, expense ledger, or forward-looking spending plan if you have one. The more detailed, the better. Annual expenses broken out by major categories such as housing, clothing, entertainment, medical, groceries, dining out, etc. Also include a description of large periodic "off-budget" items such as: "we plan to buy a new car every 7 years for approx. $35,000."

  • Upload Social Security wage history (new and continuing clients who are not yet collecting SS benefits; 10 min. est. completion time). Click the link for step-by-step instructions to easily export your SS wage history. If married, please complete this for each spouse (even if one or both of you has no work history).

  • Link your non-managed accounts through the FFP planning portal (if you do not have or have forgotten your login credentials, please email me).
