Forms for new and renewing clients to complete:

  • Pre-planning profile (prospective clients; 3 min. est. completion time). Then schedule a prospective client discovery meeting on my calendar.

  • Plan data input (new clients; 15-30 min. est. completion time).

  • Risk profile (new and renewing clients; 5 min. est. completion time. Each spouse should complete independently).

  • Upload your most recent filed tax return (new and renewing clients; include form 1040, all schedules and worksheets).

  • Plan review (renewing clients; 5-10 min. est. completion time).

  • Securely upload documents (new and renewing clients; 5 min. est. completion time). Most recent year-end paystub(s), investment account statements (employer retirement plans, brokerage accounts, IRAs, mutual funds), 401(k) investment options, employee benefit summaries, insurance statements and/or summaries (not the entire policies please!), other relevant financial documents. Note: I do not need statements for accounts held at Folio Investing.

  • After you have submitted all of the above information, book a time on my calendar for a plan presentation or review meeting (new and renewing clients; please allow 2 weeks for me to thoroughly prepare).
